I am currently going through a divorce that will be finalized on 05SEP2013 and my (soon to be) ex-wife was suppose to get this Shared Expense Plan Completed so that we didn't have to worry about her paying me child support (since I have my children)... she has failed to do so. So here I am, up at 0100, trying to do what I can to figure out how to write this agreement or something so that I don't have to worry about this on Thursday. We went for an initial hearing and the judge did say we have to have this paper, so I am kind of worried now. PLEASE PLEASE PLEASE, someone help me figure this out. I need an example to be able to write from to make our own, but cannot find a single one online.

You simply need to look at the examples at the end of the child support guidelines (which can be downloaded here). I believe one of the examples in the appendices at the end offers an expense sharing plan. Basically it just states who pays for what - make sure you cover all the bases.
